VACCINATION CLINIC 

In conjunction with SENATOR CAPPELLETTI'S OFFICE,  Eric's RX Shoppe is happy to partner to provide a vaccine clinic for ages 3+.  Details of the event are below:

Date: Wednesday, November 19th from 4 PM - 7 PM

Where:  Dixon Center (Villanova University Cabrini Campus - 1300 Upper Gulph Rd, Wayne, PA 19087)

 

Vaccine Offered:​​​​

• Flu Vaccine - Ages 3-64

• Flu Vaccine - Ages 65+ (High Dose)

• SpikeVax by Moderna Covid 19 Vaccine - Ages 3-11​​

• mNEXSPIKE by Moderna Covid 19 Vaccine - Ages - 12+​

FOR ALL COVID VACCINES:
I understand the benefits of vaccination and possible side effects. I agree to receive the vaccine as part of a shared decision with my healthcare provider (pharmacist).
